IMPORTANT: The ministry has mandated that all patients produce their Ontario Health Card prior to treatment.

Updates may be required because:

• Your Ontario Health Card is unswipable (the magnetic strip does not work)
• Your Ontario Health Card is invalid

It is your responsibility to resolve these problems.

Without a valid Ontario Health Card, you will be responsible for the charges associated with your visit.

When you receive a new OHC number, please call South Huron Hospital at (519) 235-2700 Ext. 5154 as soon as possible so that we can update your record.
