COMMUNITY RESOURCE GUIDE
Social Work
Hours of Operation:
Full-time services supporting in-patients and outpatients of SHHA
Social workers at community-based settings address the following needs:
- adjustment to illness and disability
- caregiver distress
- communication difficulties, challenging social interactions and stressful situations and behaviours
- individual psychosocial issues that impact on emotional, behavioural, physical, spiritual cognitive and social well being
- assess abuse and neglect
- advocate and facilitate referrals to community agencies
- enhance parenting skills
- discharge planning
Referrals to the Social Worker come from the family, caregiver, health care providers and through individual requests. Hours of service are Mondays through Thursdays 8:30 a.m. - 4:30 p.m.
