About The South Huron Hospital Association (SHHA):

We serve the Municipality of South Huron and its adjacent communities—a catchment area of approximately 19,000 residents. The hospital is located in the village of Exeter, approximately 30 minutes north of London, and provides 24/7 emergency services in addition to acute, rehabilitation and complex continuing care inpatient services and various diagnostic and ambulatory services. Our staff also supports family practice and walk-in care at the South Huron Medical Centre. As a small rural hospital, we cherish the relationship we have with our community and pledge uncompromising standards to those we serve.

 

Position Overview:

As a member of the Diabetes Education Team, the nurse educator will plan, develop, implement and evaluate education services within the Huon-Perth Diabetes Program.  The DNE will increase awareness of diabetes and associated risk factors within the community at large and is a professional resource for health care providers in the area. 

 

Job Duties:

  • Initiates and collaborates with patients to collect data for screening, complete an in-depth assessment, develop and implement a diabetes care plan, provide nutrition and diabetes education, and provide ongoing monitoring, evaluating and reassessment
  • Identifies priorities, target dates and strategies to contribute to the individualized education programs that achieve the optimal outcome for each patient 
  • Acts as a consultant and resource to other health care providers regarding diabetes education and management
  • Participates in evaluation process of diabetes and education services 
  • Provides community awareness programs to the general public
  • Inputs and utilizes data into D-CID program for statistical collection
  • Functions according to the Standard for Diabetes Education in Canada and follows Clinical Practice Guidelines

Requirements:

  • Registered Nurse holding a current Certificate of Competence with the College of Nurses of Ontario
  • Certified Diabetes Educator (CDE)
  • Current Police Check/Vulnerable Sector Screen and immunization status required upon hire.

Experience, Skill and Abilities: (required)

  • Minimum 1-year experience in diabetes education
  • Experience in adult education and teaching 
  • Experience in individual/group counselling and program management 
  • Advanced assessment, critical problem solving and decision-making skills
  • Excellent interpersonal and communication skills
  • Computer competency using MS Word, Excel, Publisher, One Chart, PS Suite
  • Experience with injectable therapy, glucose pattern management and continuous glucose monitoring devices
  • Knowledge of nutrition management in diabetes care

Immunization Records: Successful candidates must provide evidence of fully completed COVID-19 vaccination status prior to your first day of work.
